Steps to Admission

At Homewood Care we take the time needed to make sure we understand and learn about the specific needs of your loved ones.

With every resident that joins the Homewood family, we welcome their family and friends, whether its permanent or respite accommodation.

We want you to feel happy with your decision and we have plenty of experience and assistance available to make your move into Homewood as smooth as possible.

Walking you through each step

To enter aged care, you need an ACAT (Aged Care Assessment Team) assessment, which will evaluate whether you need high care respite, permanent admission or dementia-specific accommodation. Your doctor, social worker, loved one, nominated representative or you may arrange an assessment.

ACAT will make an appointment to visit you at home or in hospital to do the assessment. You can have a trusted friend, family member or carer with you. They will ask you questions about your daily living activities and the help you need or have in place. They may also talk to your doctor about your medical history. When all the information is collected, they’ll explain their decisions and give you a copy of the assessment, which tells you what type of services you’re eligible and approved for.

The next step is for you or your family to visit aged care homes. We’d love to offer you a tour and talk about your needs. We hope you’ll like Homewood, and if we’re your choice, please leave a copy of your assessment and contact details with us. We’ll provide you with a “Request for an income and asset assessment” booklet, which you can fill out and present to Centrelink while you’re waiting for a place.

You’ll also need to ask your doctor if they’re able to continue your care when you’re living at Homewood. This is important as your doctor will need to provide details of all your medications when you enter. If your doctor is not able to continue your care, we’ll give you a list of doctors you can choose from. A doctor is required to regularly review the care and medications you receive at Homewood. Allied health and specialised services may be also be accessed during your stay.

How to apply for admission


  1. Make an enquiry and book a tour with Homewood Care.
  2. Visit Homewood Care with a copy of your ACAT assessment if its available.
  3. When you visit Homewood or make an enquiry over the phone, we will talk to you about your needs and the admission process.
  4. We will discuss your application, provide you with an Information Pack which includes the Government request for Income & Assessment booklet to complete and return to Centrelink.
